When setting up the plugin, a webhook is registered with Mailchimp. This can be seen by an entry in the list view under Settings -> Webhooks:
If the subscriber unsubscribes via the link in the footer of the newsletter or if an admin sets the subscriber's status to Unsubscribed in the list, this information is also transferred to Shopware and the subscriber is also unsubscribed there.
